Filling in this form does not book an appointment with the practice. Before your consultation you'll need:

1. An appointment with our rooms — call or email us to arrange a time.
2. A current GP referral to claim a Medicare rebate — your GP can send this through to us before your visit.

Once your appointment is confirmed, completing the relevant questionnaire ahead of time helps us make the most of your consultation.
